Facilitator Trainings for Sexuality Education in the PNWD
Levels 7-9 & 10-12  July 12-14, 2013 Hosted by Unitarian Universalist Church of Spokane, WA Sponsored by the Pacific Northwest District of the UUA When: Friday, July 12 (1:00 p.m.) – Sunday, July 14 (4:30 p.m.)
Registration begins at noon. To be certified you must attend the full weekend. This includes the OWL curriculum for the level you will be teaching; plus, Sexuality and Our Faith. These can be purchased at the UUA Bookstore www.uuabookstore.org. Accommodations: Home Hospitality requests must be made prior to June 28. Please check on your registration form. Meals provided on-site: Dinner Friday and Saturday, simple breakfast Saturday and Sunday, lunch Saturday and Sunday as well as snacks. Please bring a snack to share for the snack buffet. Registration Information: Register via mail with this form or online at the PNWD website. No email, phone or on-site registrations accepted. Maximum number of participants is 24. If there are fewer than 12 registrants by June 28 the training may be cancelled. Registrations received by June 28 pay $250. Registrations received between June 29 and July 5 pay $285. NO REGISTRATIONS ACCEPTED AFTER July 5. Make checks payable to “PNWD-UUA” with “Spokane Jr-Sr OWL” in the memo. Please add $5 to Canadian checks and money orders to cover processing surcharge. Registration and full payment must be received at PNWD by July 5. *Refund Policy: All cancellations prior to the final registration deadline, July 5, will be subject to a $15.00 processing fee. Cancellations after July 5 or a no-show may forfeit entire application fee, except in cases of hardship. Please submit cancellation requests in writing to registrar@pnwd.org.

Friday evening dinner, as well as lunch. Saturday and Sunday, are provided. Please bring a snack to share for the snack buffet. To be certified you must attend the full weekend.
